By Shane Reeve, CEO, TwelveStone Health Partners

I once heard a story about a farmer standing at the edge of his field at the end of harvest.
The last rows had just been cut. The sun was setting, casting that warm golden glow across the land. Dust hung in the air behind the combine, and the grain trucks were full.
The farmer removed his cap, wiped the sweat from his forehead, and stood quietly for a moment—thanking God for the end of a hard season.
Yes, the farmer planted.
Yes, the farmer worked.
But he didn’t send the rain.
He didn’t warm the soil.
He didn’t cause the seed to grow.
Harvest is the moment when hard work meets gratitude.
That image has stayed with me because it perfectly reflects what many of us experience at important moments in our journey—especially when we pause long enough to recognize the results of years of dedication, teamwork, and faith.
At TwelveStone Health Partners, our theme for 2026 is “Harvest & Horizon.” It is a phrase that captures two powerful truths about where we are as an organization and where we are going.
A Time to Recognize the Harvest
Over the past decade, TwelveStone has grown from an idea into a nationally recognized leader in specialty pharmacy and infusion services. Along the way, we’ve built a team of compassionate professionals, created access to complex therapies for patients across the country, and established partnerships with providers who trust us to support their patients through some of the most difficult moments of their healthcare journeys.
None of this happened overnight.
It required years of planting, nurturing, and persistence. It required countless hours of clinical expertise, operational excellence, and a shared commitment to doing the right thing for patients and providers alike.
The harvest we are seeing today—the growth, the impact, the lives touched—is the result of that faithful work.
And it is worth pausing to recognize it.
- Every patient served.
- Every therapy coordinated.
- Every partnership strengthened.
Those outcomes represent the collective effort of an incredible team that continues to show up each day ready to make a difference.
I want to personally thank everyone for the extraordinary work accomplished in the first quarter and throughout the past year. Your dedication is the reason TwelveStone continues to grow and serve more patients every day.
Looking Toward the Horizon
But harvest is never the end of the story.
Farmers know that as soon as one season ends, another begins. New fields must be prepared. Seeds must be planted again. The horizon always holds the promise of what comes next.
That is why the second half of our theme—Horizon—is just as important.
While we celebrate what has been accomplished, we also lift our eyes forward. The healthcare landscape continues to evolve, and the need for specialized care coordination, infusion services, and patient-centered pharmacy solutions has never been greater.
Patients are navigating increasingly complex therapies. Providers are under pressure to deliver better outcomes with fewer resources. Health systems are searching for partners who can deliver both clinical excellence and operational efficiency.
These challenges represent opportunities for TwelveStone.
- Opportunities to expand access to care.
- Opportunities to innovate in specialty pharmacy and infusion services.
- Opportunities to reach more patients who depend on the kind of coordinated care our team delivers every day.
The horizon is wide, and the work ahead is meaningful.
Gratitude for the Journey
As we move forward through 2026, I encourage all of us to keep both ideas in mind: Harvest and Horizon.
Take time to appreciate the progress we’ve made together. Recognize the impact of the work being done across every part of the organization. Celebrate the milestones that remind us how far we’ve come.
But also keep looking forward.
- There are still fields to plant.
- There are still patients to serve.
- There are still new opportunities waiting beyond the horizon.
And I believe the best chapters of the TwelveStone story are still ahead.
Thank you for the work you do every day to make that future possible.
