TwelveStone provides two primary types of home-based support: enteral nutrition for patients who need tube feeding and home infusion therapy for patients receiving prescribed IV medications. Each service includes coordinated clinical support, supplies, and patient or caregiver education based on the prescribed plan of care.
Both services are designed to support continuity of care at home while helping patients and caregivers understand their equipment, supplies, and treatment routines.
When home treatment is not the most appropriate setting, TwelveStone can coordinate care across its network of more than 30 infusion centers throughout the Southeast. Our specialty pharmacy supports 62 medications, allowing our team to coordinate a range of infusion therapies across home and outpatient settings.

What to Expect When Starting Home Infusion or Enteral Therapy
After your physician sends a referral, TwelveStone coordinates the clinical, insurance, pharmacy, and delivery steps needed to begin home therapy. The exact process varies by therapy and insurance requirements, but generally includes:

- Referral and prescription review. Your prescribing provider sends the referral and therapy orders to TwelveStone so our clinical team can review the prescribed treatment and care needs.
- Benefits verification and authorization. Our team reviews your insurance benefits and coordinates any required prior authorization before medications, formulas, or supplies are scheduled for delivery.
- Medication, nutrition, and supply coordination. Depending on your therapy, our pharmacy and care teams prepare or coordinate prescribed medications, enteral formulas, pumps, and other necessary supplies.
- Home delivery and training. TwelveStone coordinates delivery of your prescribed therapy and supplies. When nursing support is part of the plan of care, a clinician provides guidance on setup, administration, equipment use, and caregiver responsibilities.
Before therapy begins, prepare a clean area for treatment and enough space to store medications, formulas, equipment, and supplies as instructed. Depending on your prescribed therapy, your delivery may include enteral formulas, feeding or infusion pumps, administration supplies, or medications. Your care team will also explain ongoing monitoring, supply refills, and whom to contact with questions to support continuity of therapy.

Safety and Support During Home Therapy
Patient and caregiver education is an important part of home therapy. When applicable to the prescribed plan of care, TwelveStone provides training on equipment use, medication or formula handling, hygiene and setup procedures, and when to contact the clinical care team for support.
What If You Have an Equipment Problem?
If a feeding or infusion pump alarms or does not appear to be working correctly, follow the instructions provided during your training and in the equipment manufacturer’s guidance. If the problem continues or you are unsure how to proceed, contact your clinical care team using the support instructions provided with your therapy.
Before home therapy begins, make sure you know whom to contact with clinical questions, supply concerns, or equipment issues. Keep the support information provided by your care team accessible, and seek emergency medical care for urgent or life-threatening symptoms.
